Questionnaire

Please fill out a separate questionnaire for each dog you are registering. We apologize for the length of the questionnaire, but the more information we have about the lifestyle your dog leads at home, the more we can customize his stay so that he feels completely at ease. For example, it is important to know if “cricker-snack” means a tasty treat, “go pooh-pooh” means relieve him/herself, or if “get Mr.wooley-man” means fetch your fleecy toy for play. Each family has their own unique words, & we want to simulate the home environment as closely as possible.

On the bright side, once you have filled out this questionnaire we will keep it on file & will refer to it whenever necessary, so that you only have to answer these questions once. Of course if any of the information changes over time, please be sure to let us know so that we can update your dog’s file.
